Rank: (we expect you to make this in accordance with your age. we don't want a 7 year old Sannin right?)
Appearance: (what your character looks like. It can be either in words or picture. But remember, a picture is worth a thousand words. not literally though. XD)
Strengths: (state where your character excels, may it be in chakra control, taijutsu, stamina etc.)
Weaknesses (state where your character totally is weak at. Remember, the number of weakness must be equal or more than the number of strengths)
History: (what happened to your character in his past? state it here. be sure to make decent amount of sentences. No 1 to 3 crappy sentences. make it at least presentable)
